Notice regarding protected health information.
This page explains the privacy roles of MHP Total Wellness, the telehealth platform, licensed clinical providers, pharmacies, laboratories, and other authorized service partners.
1. MHP Total Wellness is not your healthcare provider
MHP Total Wellness is a non-medical administrative and technology business. It does not diagnose medical conditions, provide clinical treatment, or make prescription decisions. Medical services are provided by independent licensed healthcare providers through the telehealth platform.
2. Who may handle your information
Depending on the services you request, your personal and health information may be collected, used, maintained, or transmitted by:
- The licensed healthcare provider or professional medical entity responsible for your clinical care;
- The secure telehealth and electronic health record platform;
- Participating licensed pharmacies;
- Laboratory or diagnostic service providers;
- Payment processors, fulfillment providers, and shipping partners where permitted;
- MHP Total Wellness for authorized administrative functions; and
- Other service providers permitted by contract and applicable law.
3. Protected health information
Protected health information, often called PHI, generally includes identifiable information about your health, medical history, treatment, prescriptions, payment for healthcare, or other healthcare services. PHI submitted for clinical care is maintained under the authority of the applicable healthcare provider or covered entity.
4. How information may be used
Subject to applicable law and the governing provider's Notice of Privacy Practices, health information may be used or disclosed for purposes that include:
- Evaluating your medical information and providing treatment;
- Coordinating prescriptions with licensed pharmacies;
- Arranging laboratory or diagnostic services;
- Processing payment and administering program charges;
- Operating, supporting, auditing, and securing the telehealth platform;
- Responding to patient-support and fulfillment inquiries;
- Meeting legal, regulatory, licensing, and public-health requirements; and
- Other purposes authorized by you or permitted by law.
5. MHP Total Wellness access to health information
MHP Total Wellness may receive or access limited information when necessary to perform authorized administrative functions such as enrollment support, payment coordination, customer service, order status, fulfillment coordination, and issue resolution. MHP Total Wellness does not use health information to direct provider judgment or make clinical decisions.
6. Your privacy rights
Your rights may include the ability to request access to your medical records, request corrections, request restrictions, request confidential communications, receive an accounting of certain disclosures, obtain a copy of the applicable provider's Notice of Privacy Practices, and submit a privacy complaint. The exact rights and procedures are described in the Notice of Privacy Practices issued by the healthcare provider responsible for your care.
7. Requests for medical records or privacy rights
Requests involving medical records, amendments, restrictions, confidential communications, or accountings of disclosures should generally be directed to the healthcare provider or professional medical entity responsible for your treatment through the secure telehealth platform. MHP Total Wellness customer support may help route an administrative request but may not have authority to fulfill a clinical-record request.
8. Security
Reasonable administrative, technical, and physical safeguards are used to protect information. No electronic system can guarantee absolute security. Patients should protect account credentials, use secure devices and networks, and promptly report suspected unauthorized account access.
9. Communications
Ordinary email and text messaging may not be appropriate for sensitive medical information. Clinical information should be submitted through the secure telehealth platform whenever available. Do not send emergency information by email.
10. Privacy complaints
You may submit an administrative privacy concern to MHP Total Wellness at mhpllc.co@gmail.com or by calling (720) 270-7131. Complaints involving a healthcare provider's use or disclosure of PHI should also be submitted directly to that provider using the contact information in its Notice of Privacy Practices. You will not be retaliated against for submitting a good-faith privacy complaint.
11. Changes to this notice
This notice may be updated to reflect changes in services, partners, law, or privacy practices. The effective version will be posted on this website.
