Telehealth Consent and Disclosures

Nature of telehealth
Telehealth uses electronic communications and technology to allow a licensed healthcare provider and patient to exchange health information and provide clinical services when they are in different locations. Encounters may be synchronous, asynchronous, or both, where permitted.
Potential benefits
Potential benefits include convenient access, reduced travel, efficient information exchange, and access to licensed professionals.
Material risks and limitations
Risks may include technology failure, interruptions, incomplete information, privacy or security incidents, delayed care, and limitations on a provider's ability to perform a physical examination. A provider may determine that in-person care is required.
Independent clinical judgment
All diagnosis, treatment, prescribing, and follow-up decisions are made solely by the licensed healthcare provider. MHP Total Wellness does not control or influence clinical decisions.
Location and identity
You may be required to verify your identity and current physical location. The provider must be appropriately licensed in the jurisdiction where you are located at the time of the encounter.
Prescriptions
No prescription is guaranteed. Prescriptions are issued only after provider review and only when medically appropriate and legally permitted.
Privacy
Electronic systems include privacy and security safeguards, but no system can eliminate every risk. Use a private location and secure device whenever possible.
Emergency limitations
Telehealth through this service is not appropriate for emergencies. Call 911 or seek immediate emergency care.
Consent
By proceeding through the patient portal and affirmatively accepting the telehealth consent presented there, you acknowledge the nature, benefits, risks, and limitations of telehealth and consent to receive services through electronic communications, subject to your right to withdraw consent where permitted by law.
